JACGP Community Service Awards
Japanese American Community Graduation Program’s Leo Goto Service Award 2012
- ELIGIBILITY
- A Candidate must meet one of the following:
- A graduating high school senior residing in the Denver Metropolitan area who is either an American citizen of Japanese ancestry or an American citizen who’s legal parent(s) is/are of Japanese ancestry, or
- A graduating high school senior of Japanese ancestry residing in the Denver Metropolitan area who has permanent residence status (INS documentation) in the United States, or
- A graduating high school senior who has a parent who is an active member of: Mile-Hi Chapter Japanese American Citizens League, Simpson United Methodist Church, Tri-State/Denver Buddhist Temples, Denver Nisei Bowling Association, Brighton Japanese American Association, Japanese Association of Colorado, Nisei Veterans Heritage Foundation, Japanese Firms Association of Colorado, Longmont Buddhist Temple, or Fort Lupton Japanese American Citizens League.
- Letters of “Attestation of Volunteerism in the Japanese American Community” totaling a minimum of twenty four (24) hours during the previous four years. Must be documented on attached form.
- Completed application form.
- A Candidate must meet one of the following:
- REQUIRED FORMS
- Completed Community Service Application Form
- Nomination Form with one or two letters of recommendation.
- Letters of “Attestation of Volunteerism in the Japanese American Community” totaling a minimum of twelve hours during the previous four years. Must be documented on attached form.
All of the above three listed documents are required to qualify for the judging process. Any packet missing any of these documents will be disqualified from the judging process.
- JUDGINGThe final decision of the judges is based upon community service/personal sacrifice. Academic achievement is not a consideration. A student cannot receive both a Community Committee Award and a Community Service Award but can apply for both. The Community Service awards may not be given annually.
- DEADLINEALL MATERIAL MUST BE POSTMARKED ON OR BEFORE APRIL 6, 2012. There will be NO EXCEPTIONS to this deadline. Certificate of mailing from the Post Office is required. Hand delivered applications will not be accepted.Mail completed application to:
